Does Medi-Cal Cover Braces for Adults?

By Ross Seymour of Free Dentist Office, a directory of dentists who take Medi-Cal, built from the state's public provider data. It is not run by any state agency.

No, not for adults. Medi-Cal covers braces only for people under 21 whose bite problem is serious enough to count as a health issue. The state's Smile California site lists braces for teens who qualify and leaves them off its list for adults 21 and older, checked in October 2026.

  1. Age limit. Under 21 only, per Smile California's teen and adult lists
  2. Who qualifies. Young people with a serious bite problem, measured by a set score
  3. Adults. Pay privately, or look for lower cost options
  4. Find an orthodontist. Orthodontists on this site

The Age Line

Medi-Cal treats orthodontics, which means braces and similar treatment, as a health benefit for young people, not adults. Smile California, the state's Medi-Cal Dental site, lists orthodontics for those who qualify on its page for teens aged 13 to 20. Its page for adults 21 and older has no braces on the list. We checked both in October 2026.

A 2005 state hearings note that quotes the Denti-Cal manual says this care is limited to Medi-Cal members under 21 who have what is called a handicapping malocclusion. That is a bite problem bad enough to affect health or function, not just looks.

After 21 the benefit ends, according to those same state sources. That is true even for adults with a severe bite, so it is worth knowing the age line if you have a teenager.

Comparison of Medi-Cal Dental lists. Teens 13 to 20 get orthodontics for those who qualify, molar sealants, exams and cleanings every 6 months, fillings, crowns and root canals. Adults 21 and older get no braces, exams and cleanings every 12 months, fillings, crowns and root canals, up to 1,800 dollars a year.
Braces are on the teen list, not the adult list. Smile California, checked in October 2026.

How the Under 21 Decision Gets Made

An orthodontist who works with Medi-Cal examines the young person and scores the bite on a set scale called the HLD index, according to that same 2005 note. They take study models of the teeth. If the score is high enough, or certain severe conditions are present, the office asks the program for approval.

Many young people with crooked teeth do not score high enough. If your child is turned down and you disagree, you can ask how to request a hearing. Our guide on dental care for kids on Medi-Cal goes into this in more detail.

If You Are 20, Move Fast

This is the one case where timing really matters. If you are close to 21 and think you might qualify, book an orthodontic exam now. Approval takes time. Ask the office what happens if you turn 21 partway through the process, so you know where you stand before treatment starts.

Parents of teenagers can use the same advice. If a dentist has ever mentioned a bad bite, ask for an orthodontic referral while your child is still well under the age line.

Options for Adults

Most adults who want braces pay privately. Ask orthodontists for a written price for the whole treatment, including retainers and follow up visits, and ask whether they offer a monthly payment plan.

Dental schools with orthodontic programs may offer treatment at a lower price, with a longer wait. Ask whether they take adult patients and how they decide who to accept.

Be careful with mail order aligner kits. Straightening teeth moves bone and roots. An exam by a licensed dentist first is the safer route, especially if you have gum disease or fillings and crowns.

Why Looks and Health Are Treated Differently

Medi-Cal pays for care that keeps a mouth working. Straight teeth for appearance alone falls on the other side of that line, the same way whitening does. The under 21 benefit is meant for a bite bad enough to affect health or function.

For adults the program focuses on keeping teeth healthy and replacing ones that are lost. If your bite causes real pain, trouble chewing or damage to teeth, tell a dentist. Some of those problems have covered treatments that are not braces.

Talking to a Dentist About Your Bite

Describe what bothers you in plain words. My teeth do not meet when I chew. My jaw clicks and hurts. I keep chipping the same tooth. That helps the dentist decide whether the problem is cosmetic or something that needs treatment.

Ask what the options are at each price, including doing nothing for now. A good dentist will explain which ones Medi-Cal covers and which ones you would pay for.

What Medi-Cal Still Covers for Adults

Medi-Cal Dental still covers the care that keeps teeth healthy for adults. Smile California lists exams, cleanings, x rays, fillings, tooth removal, root canals, crowns and dentures, and says Medi-Cal will pay up to 1,800 dollars a year for covered adult dental work, checked in October 2026. Crooked teeth can be harder to clean, so regular cleanings matter more, not less, if braces are not an option.

Smile California also says that starting July 1, 2027, some adult members will lose full dental benefits based on immigration status. Emergency dental care stays covered.

Coverage rules can change. To confirm what Medi-Cal covers for you, call Medi-Cal Dental at 1-800-322-6384. The call is free.

Before You Spend a Dime

Does Medi-Cal cover braces for adults?

No. Smile California lists braces for teens who qualify and not for adults 21 and older, checked in October 2026. That holds even for adults with a severe bite.

Does Medi-Cal cover Invisalign?

Not for adults. For people under 21 whose bite qualifies, ask the orthodontist which kinds of treatment Medi-Cal will approve before you count on clear aligners.

How does Medi-Cal decide if a child gets braces?

An orthodontist scores the bite on a set scale called the HLD index and takes study models. If the case qualifies, the office asks the program for approval.

Where can adults get cheaper braces?

Ask orthodontists for written prices and payment plans, and ask dental schools with orthodontic programs whether they treat adults at a lower cost.
