Does Medicare Cover Dental? What Seniors Can Use Instead
By Ross Seymour of Free Dentist Office, a directory of dentists who take Medi-Cal, built from the state's public provider data. It is not run by any state agency.
In most cases, no. Medicare.gov says Original Medicare does not cover routine dental care like cleanings, fillings, pulling teeth, dentures or implants. Some Medicare Advantage plans add dental benefits, and if you also have Medi-Cal, Medi-Cal Dental covers most routine work.
- Original Medicare. No routine dental care
- The exceptions. Dental care tied to certain covered medical treatment
- Medicare Advantage. Some plans add dental, and it varies by plan
- Have Medi-Cal too. Medi-Cal Dental covers most routine work
What Original Medicare Leaves Out
Medicare.gov says that in most cases Medicare does not cover dental services like routine cleanings, fillings, tooth extractions, or items like dentures and implants. We checked that page in October 2026. If you only have Original Medicare, Parts A and B, you pay the full cost of that care yourself.
This surprises a lot of people at 65. Plenty of workers had dental coverage through a job and assume Medicare picks it up. It does not.
The Exceptions Medicare Does Pay For
Medicare may cover dental care when it is tied to certain medical treatment that Medicare already covers. Medicare.gov gives examples. Some dental services during a hospital stay. An oral exam and dental treatment before a heart valve replacement or an organ or bone marrow transplant. Pulling a tooth to treat a mouth infection before cancer treatment such as chemotherapy. Dental problems that come up with head and neck cancer care. Dental exams before and during dialysis for kidney failure.
For covered Part B dental services, Medicare.gov says you pay 20 percent of the Medicare approved amount after you meet the Part B deductible, as of October 2026. If you think your case fits, ask the doctor treating your medical condition to coordinate with the dentist.
Medicare Advantage Plans
Medicare Advantage plans, also called Part C, are sold by private companies. Medicare.gov says plans may offer extra benefits that Original Medicare does not cover, like some vision, hearing and dental services. What they cover, how much they pay each year and which dentists you can see all vary by plan.
If dental matters to you, check the plan's dental details before you enroll. Ask whether cleanings, fillings, extractions and dentures are included, what the yearly limit is, and whether your dentist is in the network.

Questions to Ask a Plan About Dental
Is dental included, or is it an add on that costs extra each month. Which services are covered, and at what share. Is there a yearly limit on what the plan pays, and what happens once I reach it. Do I have to use dentists in your network. Are dentures covered, and how often can they be replaced.
Get the answers from the plan's own documents, not only from a sales call. Plans change their benefits from year to year, so check again before each enrollment period.
If You Have Both Medicare and Medi-Cal
Some seniors in California qualify for both. Smile California, the state's Medi-Cal Dental site, says that even if you have Medicare or Medicare Advantage, Medi-Cal can cover dental benefits Medicare does not. Its list for seniors includes exams, cleanings, x rays, fillings, root canals, crowns, dentures and tooth removal. It says Medi-Cal will pay up to 1,800 dollars a year for covered dental services. We checked that page in October 2026.
Smile California also says that starting July 1, 2027, some adult members will lose full dental benefits based on immigration status. Emergency dental care stays covered.
If you have Medicare and a low income but never applied for Medi-Cal, it is worth checking. You can call Medi-Cal Dental at 1-800-322-6384, a free call, with questions about dental coverage.
Low Cost Options for Seniors With No Dental Coverage
Community health centers see people without insurance. HealthCare.gov says how much you pay there depends on your income. Dental schools offer care at lower prices because students do the work with an instructor checking. The California Dental Association Foundation runs CDA Cares Connects, volunteer dental events that give care at no charge, so check its page for the next one.
Dental Lifeline Network runs a donated dental program for people over 65, people with permanent disabilities and people who need dental care for a medical reason, when they cannot afford it. As of October 2026, its California page says all counties are closed to new applications because of long waiting lists, so check back before you count on it.
Do Not Skip the Dentist Because of Medicare
Mouth problems in older adults often start without pain. A cracked filling, a loose denture or a sore that will not heal is worth a visit. If cost is what stops you, start with a sliding scale clinic and ask what a basic exam costs before you go.
If you wear dentures, have them checked when they start to rub or slip. A poor fit can cause sores and make eating hard, and both are easier to fix early.
Before You Spend a Dime
Does Medicare cover dental cleanings?
No. Medicare.gov says Original Medicare does not cover routine cleanings, fillings, extractions, dentures or implants in most cases. Some Medicare Advantage plans include cleanings.
Does Medicare cover dentures?
Original Medicare does not. Some Medicare Advantage plans include them. If you also have Medi-Cal, Medi-Cal Dental covers dentures.
Does Medicare cover tooth extraction?
Not routine extractions. Medicare may cover pulling a tooth when it is part of covered medical care, such as treating a mouth infection before cancer chemotherapy.
Where can a senior get free dental care?
Try Medi-Cal if your income qualifies, a community health center that charges by income, a dental school clinic, or a free event such as CDA Cares Connects. Dental Lifeline Network's donated care program was closed to new California applicants when we checked in October 2026.
