Planning Inside a Benefit, lesson 2 of 2

Case Presentation When the Patient Pays Nothing

The entire case acceptance industry is built on overcoming a cost objection. Remove the cost and the real obstacles are time, fear and whether they believe you. Those need a different conversation, and the scripted one actively gets in the way.

  1. Not the money. Cost is not the objection here
  2. The real ones. Time off work, transport, fear, past experience
  3. Ask. What would make this hard for you
  4. Be concrete. How many visits, how long, what it feels like

Why the Standard Advice Misfires

Financing options, payment plans, and the techniques for handling a price objection are answers to a question this patient is not asking. Delivered anyway, they sound like a script, and a script is the fastest way to lose somebody who has been talked at by institutions all week.

Worse, the framing can imply the care has a price they are being spared, which shifts the conversation to gratitude. That is not a good footing for a decision about their own body.

Drop it and ask a better question, which is what would actually make this hard for them.

What the Obstacles Usually Are

Time. Three appointments is three days of arrangements, and for somebody on an hourly wage it may be three days of lost pay. That is a real cost even when the treatment is free.

Transport. How they get here, and whether the answer changes if they are numb or sore afterwards.

Fear, often from a previous experience of dentistry that went badly or was delivered without much care. This is common in people who have had interrupted access for years.

Belief. Whether they think it will work, which is heavily shaped by whether previous promises made to them were kept.

The Conversation That Works

Be concrete about the shape. How many visits, how long each one is, what they will feel, what they can eat afterwards, whether they can go back to work. Vagueness reads as evasion to somebody who is already unsure.

Say what happens if nothing is done, once, factually and without drama. People make good decisions with real information and poor ones under pressure.

Ask them to tell you the plan back. This catches misunderstanding that a yes hides, and it works far better than asking whether they have questions.

Give them a way to come back if they say no today. A door left open is worth more than a decision extracted while somebody is uncomfortable.

Respect Is the Mechanism, Not the Manners

This population is used to being processed. An office that explains, gives real choices and does not hurry is unusual enough to be remembered, and the practical result is people who come back and who tell others.

That is not a soft benefit. In a practice whose economics depend on completion and recall, it is the economics.

Before You Spend a Dime

Do case acceptance courses help here?

The parts about listening and being concrete do. The parts about handling a price objection are aimed at a different practice and can do harm if delivered as written.

How do we handle someone who keeps saying they will think about it?

Ask what specifically would make it hard, and listen. It is usually a practical obstacle you can help with rather than hesitation about the treatment.

Should we explain what the program pays us?

There is rarely a reason to, and it moves the conversation onto money, which is exactly where it does not belong here.
