In the Office's Own Words

The CDS Foundation Dental Clinic is a free dental clinic for patients who qualify for services. Patients must provide proof of household income at or below 200% of the Federal Poverty Level to qualify for care. We provide basic dental care, and acrylic partials and dentures. Dental care is provided by volunteer dentists who close their own offices and come to the clinic to volunteer their time and expertise to serve the public.

Before You Call

Say what coverage you have, Illinois Medicaid or none, and ask two things. Do you take it for new patients, and what will the first visit cost me. Offices change which plans they take, so the answer on the phone beats anything written here.
