CHC provides care to all individuals, regardless of insurance status or ability to pay, including Medicare, Medicaid, and CHIP. No one is denied access to services due to inability to pay; a discounted/sliding fee schedule is available based on family size and income. CHC offers medical, dental, and behavioral health services for patients of all ages. Eye exams are available at our Fort Dodge and Mason City clinics.
